Convalescent Center Mission St. Inc. is a cute 53-bed skilled nursing facility located in the upper Mission district of San Francisco. We are positioned on the border line between San Francisco and Daly City (Top of the hill). We are just few blocks from the Daly City Bart station.

CCMS is certified for 24 hour skilled nursing care for Medicare, Medical and Private residents. Our business is to provide excellence in clinical care and services to our customers.

Our services include the comprehensive rehabilitation 5 days a week, and residents' activities of their choice. Fore-Rehab provides the therapy services to promote recovery process after illness, injury, or trauma. Individualized treatment and care plans are developed incorporating the goals of the resident, family and caregivers. We assist residents in increasing independence and returning to a lower level of care.
